Unzip the file. Place one of the .erf file in the folder:
Documents\BioWare\Dragon Age 2\packages\core\override

File name that has "notat" mean the face doesn't have the tattoo.

//REPLACING
Simply replace the existing .erf file in the override folder with a different one.
